I'm sceptical. I'm sure they do what they are supposed to and keep the baby's head from being flat on the mattress, but I always thought that flat head was caused less by the position the sleep in, and more by the amount of time they spend flat, and in car seats when they are awake.

We didn't have a pillow for either of my two and neither of them had any suggestion of a flat spot, so they're definitely not essential.
